© Stephan Wieland, Düsseldorf On the contrary: using a modern lipotransfer, it is now possible to perform a breast augmentation without implants if a moderate breast augmentation is desired and a silicone implant is not required.
Alternatively, a so-called “hybrid” procedure (Hybrid breast augmentation in which we combine a silicone implant with a fat tissue transfer ) can be applied. In this case, a soft and very natural breast fullness and shape can be achieved, providing you with the advantages of both procedures.
It is also possible to permanently remove existing breast implants, replace them with autologous fat and, if desired, combine this with a breast lift. Breast augmentation with autologous fat is performed in two steps: First, the body’s own fatty tissue – usually from the abdomen, thighs or other areas of the body – is gently removed by liposuction. The processed fat is then injected into the breast to harmoniously model the volume and shape.
Current studies show that around 60 to 70 percent of transplanted fat cells integrate into the tissue and are permanently retained. The result is therefore stable and easy to assess after around six weeks.
Depending on the number and size of the donor sites, the procedure is performed on an outpatient or inpatient basis. As a rule, a moderate breast augmentation of around half a cup to one and a half cup sizes can be achieved with one procedure. A repeat procedure is possible after six months at the earliest.
Can breast implants be removed and replaced with autologous fat?
Yes, it is generally possible to replace silicone implants with autologous fat – for example, if you want a more natural breast shape without a foreign body sensation. In many cases, the removal of the implants and reconstruction with autologous fat can be performed in one procedure.
Whether this makes sense in your particular case will be clarified in your detailed personal consultation. We will analyze the position and condition of the existing implants and whether sufficient fatty tissue is available for the transplant. Depending on the initial situation, a breast lift may also be advisable to achieve a harmonious result.

How are the fat cells for breast augmentation with autologous fat obtained?
For breast augmentation with autologous fat, we remove the body’s own fat cells through gentle liposuction – usually from the abdomen or thighs. The fatty tissue is then purified, processed and injected into the breast in small quantities using fine cannulas. In this way, the desired shape and natural volume can be achieved.
We use a special procedure in our clinic: vibration-assisted lipotransfer. This modern system enables particularly gentle and even removal and transfer of the fat cells. The transplantation is precise and controlled – the risk of uneven fat distribution is significantly reduced.
For safety reasons, only the surrounding subcutaneous fatty tissue and – if necessary – the large pectoral muscle are filled during this procedure, not the mammary gland itself. As an endogenous material, fatty tissue heals well and does not cause a foreign body reaction. The risks of this method are therefore low.
What happens after the operation?
Immediately after the procedure, a special compression girdle is applied to the donor sites, which must be worn for around six weeks. For the first few days, you will wear a special lipotransfer bra.
In the first three weeks, it is particularly important to avoid pressure on the breast. It is best to sleep on your back and avoid tight clothing and sporting activities during this time. After around six weeks, you can resume your regular physical activity and sport as usual.
How long do the results of a breast augmentation with autologous fat last?
The transplanted autologous fat is the body’s own tissue – so there is a good chance that around 60 to 70 percent of the transplanted fat cells will heal permanently.
We ensure the highest possible survival rate of the transplanted cells in our clinic through particularly gentle harvesting, preparation and transfer of the fat cells.
The result is stable after about six weeks and is usually permanent if your body weight remains the same.
What are the possible risks of breast augmentation with autologous fat?
Breast augmentation with autologous fat is considered a low-risk method with very natural results – especially when performed by an experienced plastic surgeon.
As with any surgical procedure, complications or undesirable side effects cannot be completely ruled out.
A particular risk is the possible occurrence of oil cysts and calcification of the tissue, especially in the case of improper fat transplant. This risk is virtually eliminated when using our special, machine-assisted transfer technique.
